Call or email me (Alicia Dorn, LCPC) at the information provided above to schedule your free 15-minute consultation call. This allows you to openly discuss your challenges and ask questions to determine if I am the best fit to help you on your journey. Once you determine you’d like to move forward, we will schedule your intake appointment.
Intake forms will be sent to you through your therapy portal. You will be able to review, sign and upload prior to your intake appointment.
I am an “Out of Network” provider, and therefore am not paneled with any insurance companies.That means that you are responsible for the fees at the time of your appointment. Depending on your plan, a certain percentage of that may be paid to you by your insurance company. If you would like to use your insurance benefits, I would be happy to provide a Superbill receipt for you to submit to your insurance company.
Fees:
– Intake Assessment (1-hour): $200
– Therapy Session (1-hour): $175
Debit, credit and HSA/FSA cards are accepted for payment at each appointment.
I will also offer you a Good Faith Estimate as defined below at the time we begin services together.
All therapy services will be conducted virtually. Specifically, all individual therapy client portals can be located at this link “https://www.therapyportal.com/p/adorned21236/”
Family and group therapy sessions will be conducted through a secure Zoom link, which will be provided prior to the scheduled appointment.
Crisis counseling is not offered but I will provide you with crisis support information in your intake packet once your first appointment is scheduled. I am also happy to help you reach crisis support during standard business hours (9:00 AM – 6:00 PM EST).
Yes. Therapy services are offered for adolescents from ages 11-18.
Your payment method will be stored through your client portal, which consists of a secure HIPAA compliant online payment system. Your card will be charged at the time of the appointment.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate”
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate.
